I have two Rockwell  34-454 wing extension. They are heavy still in original box $100.00 and shipping. Box weighs 50 pounds. Appears to have never been used still in wrap
I never took them out of the box. If there is interest I will post photo. It says Rockwell but by then it had Delta part #. It must be for a unisaw. Box measures 27.5 by by about 3.5 inches.
I was going to check them for flatness but never did as I wanted to use them for sandpaper sharpening table.
